从研究的角度讲,具体技术会像潮流一样,一波一波地出现和盛行。每当一种新鲜技术出现时,有关领域的研究者和实践者往往趋之若鹜,原来的技术也就淡出人们的视线。特别地,面对新鲜技术,人们倾向于认为原来的技术是落后的。然而,许多技术显得并没有好坏之分,一些技术在若干年之后会被重新关注,再次流行。甚至,一些研究者和实践者在别人纷纷采用新鲜技术时,仍然使用原来的技术开展工作,并且取得卓越成就。
经颅直流电刺激(transcranial direct current stimulation, tDCS)就是这样的一种技术。20世纪70年代,tDCS技术在研究中逐渐被抛弃了。然而,30年后,人们对tDCS重新产生了兴趣。在世纪之交,两项重要研究表明,经颅微弱电流能影响人类运动皮层的激活(Nitsche & Paulus, 2000; Priori et al., 1998)。这样的研究引起对tDCS的重新评价,使tDCS再次成为考察神经调节的一种重要技术。
在小脑研究领域,tDCS发挥着独特作用。第一项小脑tDCS研究,以小脑经颅磁刺激(transcranial magnetic stimulation, TMS)非运动过程的证据为基础,考察小脑在语言工作记忆任务中的作用(Ferrucci et al., 2008)。最近10年间,小脑tDCS既用于研究小脑在运动、认知、情感过程中的作用,也用于研究治疗神经系统紊乱的临床效果。
小脑tDCS在运动功能方面的研究,主要考察小脑在运动适应和运动学习过程中的作用。研究发现,阳极小脑tDCS能够促进多种运动适应范式中的成绩(Poortvliet et al., 2018),而阴极小脑tDCS会抑制运动适应(Jayaram et al., 2012)。运动学习研究发现,阳极小脑tDCS对运动序列的获得、保持、巩固、重新获得具有促进效应(Zuchowski, Timmann, & Gerwig, 2014)。一些研究发现,阴极小脑tDCS可以抑制运动序列的获得和保持(Herzfeld et al., 2014)。
在认知过程领域,小脑tDCS的刺激效应主要表现在与工作记忆(Pope & Miall, 2012)和语言(Macher et al., 2014)有关的功能上。虽然大多数研究发现小脑tDCS影响这些认知过程,但是,tDCS对认知功能的效应要小于tDCS对运动功能的效应;同样地,不像运动过程研究所报告的阳极小脑tDCS具有促进效应、阴极小脑tDCS具有抑制效应,小脑tDCS刺激的极性依存效应在认知过程研究中也没那么明显(Oldrati & Schutter, 2018)。
考察小脑在情感加工中作用的tDCS研究不断增多。小脑参与情绪过程的证据,涉及对负性情绪面孔的再认。研究表明,阴极与阳极tDCS均可促进这个过程(Ferrucci et al., 2012)。当把阳极置于大脑左半球背外侧前额叶皮层上面,阴极置于小脑右半球上面,进行额叶小脑tDCS时,可以促进健康被试的情绪测量指标;特别地,这一研究还表明,相反的极性设置,对情绪具有类似的效应(Newstead et al., 2018)。
小脑tDCS在临床应用方面,主要探索治疗运动障碍的效果。研究表明,小脑tDCS能够改善脊髓小脑性共济失调病人的运动成绩(Bodranghien et al., 2017),可以改善左旋多巴诱发的运动障碍和帕金森氏症病人的平衡问题(Workman et al., 2020)。小脑tDCS越来越多地用来促进中风病人的康复,结果显示,小脑tDCS既能促进某些语言功能的恢复,也能促进平衡和步态运动的恢复(Ponce, Klaus, & Schutter, 2021)。
需要指出的是,在世纪之交,虽然临床上对小脑刺激的注意已经从侵入式技术转入了非侵入式技术,但是,侵入式神经调节依然用于治疗神经系统障碍病人。例如,侵入式小脑刺激仍被作为一种中风后康复疗法。中风幸存者的康复程度与许多因素有关,特别是中风慢性期康复过程中的皮层兴奋性。研究显示,小脑皮层的兴奋性能够通过对小脑深部核团齿状核的慢性电刺激而增强(Machado et al., 2009)。
总之,tDCS在21世纪重新受到青睐,研究者用它开展包括小脑tDCS方面的探索,不断有新的发现。即使非侵入式小脑刺激更为流行,侵入式小脑刺激也仍然有用武之地。其实,不仅研究技术是这样,而且研究思想、研究取向也是如此——心理学中有很多这样的情况。
显然,这里指出研究技术往往并无好坏之分,肯定不是让人抱残守缺的。对于优秀的技术,自然应当积极学习和使用。指出技术没有好坏之分,一方面,对于所谓的新鲜技术,不一定盲目追逐;另一方面,也是更为重要的,对于传统技术,也不应随便轻视,对于研究成果的评定,更不应以技术是否新鲜为标准。
