李清晨 发表于 2010-10-17 13:51 2010年10月15日,门户网站网易首页推荐了一篇题为《钟南山为什么力挺中医抗癌?》的博客文章,作者系中华医学会心身医学会分会主任委员、上海中医药大学博士生导师、教授何裕民,其中有这样一段话: 我们的临床表明:中医是可以治疗癌症的(当然,更多的情况下,考虑患者的综合利益,我们同样看重中西医学的合理配合)。三十年多、数以三四万例的诊疗经验,促使我们体会到对于许多癌症,中医学的疗效是明确的。特别是那些对西医疗法不甚敏感或者说疗效不显著的、较难治的肿瘤,如肝癌、胰腺癌、小细胞肺癌等,中医常常显示出不凡的疗效。例如,我们在对近400例晚期胰腺癌的纯中医治疗中,3年生存率超过了40%。所以说,对于一些西医无法可施的癌症病人,常常是中医药大显身手的时候。 作为一个医务工作者,看到这样医学界能有这样的突破,当然十分高兴,但是冷静下来却不免疑窦丛生,现阶段,号称癌中之王的胰腺癌的治疗真的能达到这样卓越的效果么?要是晚期胰腺癌的3年存活率能超过40%,那这癌中之王的名号岂不是徒有虚名了么? 话说这胰腺癌的厉害,真的是不能小视,由于其预后极差,被国际医学界列为21世纪的顽固堡垒 ,最新资料表明胰腺癌患者确诊后平均存活时间仅为6个月, 5年生存率仅0.4%~5%,只有2.6%~9%的病人接受了手术,平均存活时间11~20个月,其术后5年生存率为7%~25% 。癌中之王的称谓,真真非他莫属。而且令人不爽的是该肿瘤的发病率却在逐年增加,30年来西方国家已经上升了7倍,成为肿瘤死亡的第3位,国内也从上世纪60年代的15位上升到90年代的第6位。美国胰腺癌的年发病率约为10/10万,病死率占所有恶性肿瘤的第4位 。上海市胰腺癌年发病率及死亡率分别为10/10万和9.4/10万,位列肿瘤发病率及死亡率的第8位和第6位 。2008年,美国34290死于胰腺癌,新发病例37680人,尽管接受了现代医学的治疗,仍有90%的病人在诊断后一年内死亡 。 这种治疗结果,显然不能令人满意。但就是这些远未令人满意的成绩,也是百多年来一代代外科医生通过艰苦的努力和探索才取得的。 1898年意大利的外科医生Codivila为胰腺癌患者施行胰十二指肠切除术,开创了胰腺癌手术的治疗史,术后患者仅存活了21天。经过数十年的发展,一直到20世纪40年代,确立经典的Wipple术以后,胰十二指肠切除术的手术死亡率仍高达44%,该手术的复杂及凶险程度由此可见一斑。 2007年《劳动报》报道:严女士于2003年12月入住平湖第一人民医院,同月21日行胰十二指肠切除术。但是在手术中,严女士除了胰脏和十二指肠被切除外,同时被切除的还有空肠上段、半胃和胆囊。2004年12月,严女士提起诉讼,要求医院赔偿 这则报道给公众的印象是,这家医院太操蛋了,怎么多切了那么多器官呢?这不就是祸害人么?肯定大有问题!本来媒体监督批评医疗行为实属平常,可像这种连基本事实都搞不清楚的批评实在令人哭笑不得。尤其是但是二字,更是凸显了该记者的愚蠢。对于很多专业问题,顾名思义的结果,很可能与真相是南辕北辙的。 那么胰十二指肠切除术的切除范围究竟有多大?或者说《劳动报》记者错在何处呢? 就标准的胰十二指肠切除术(Wipple术)而言,其切除范围包括胰头、远端胃、十二指肠、上段空肠(小肠在解剖学上分三段:十二指肠,空肠和回肠)、胆囊和胆总管最后是消化道的重建。该记者显然是想当然地以为这个手术切除的范围只包括俩器官了。其实他只要随便找个学医的咨询一下就不至于如此,可惜有些记者也许目的本在煽风点火,惹的民怨沸腾就好,哪管真相如何呢? 也许您要恍然大悟了:哦,一次切掉这么多零件,难怪有这么大的风险啊! 时至今日,由于麻醉的完善、ICU的积极器官功能维持以及更为熟练的手术操作和有效的营养支持,已使围手术期的死亡率降低至1%左右 。 为改善手术效果,外科界一度试图从扩大手术范围入手,这确实明显提高了切除率,但手术死亡率与并发症发生率也同时大增,可存活率却未能如预想中的那样有所改善。几十年的努力付诸东流,一切似乎又回到了30年前的起点,是否有必要扩大手术仍在争论美国的外科医生们已经纷纷放弃扩大手术,而国内却仍处在追求提高手术切除率的手术热中,又兼我国尚未形成规范的统一术式,其RCT(randomized clinical trial,随机临床试验)研究结果也未能与国际接轨。复旦大学附属中山医院普外科胰腺肿瘤专业组王单松等指出,胰腺癌的外科治疗正处于感性到理性的阶段,既不主张不计后果的追求切除率、甚至扩大切除,也反对轻易放弃手术或不分具体情况一律选用传统的Whipple手术方式。针对某一特定胰腺癌患者,胰腺外科医生必须利用自己的专业知识和专业技术,遵循提高其生活质量和远期生存期的原则,根据患者具体情况制定合理的个体化治疗方案。 能坚持读到这里的朋友,恐怕已经很压抑了。虽然目前认为手术切除是胰腺癌唯一的可治愈性方法,然而85%的胰腺癌患者就诊时已属晚期或发生远处转移,手术切除率仅 10% 15% ,那么其他未能接受根治性手术的病人难道只能坐以待毙不成? 由于胰腺癌可引起黄疸、胃排空障碍、肠梗阻及腹痛,因此姑息性手术的目的即为缓解这些症状而非切除肿瘤。通常,手术室接到胰十二指肠切除术的手术预约时,护士们往往如临大敌,要精心做器械和体力方面的准备,但到了手术台上,仍有相当一部分因发现肿瘤已无法根治切除不得不行姑息性手术。因此姑息性手术反而成为胰腺癌的主要治疗手段,只是无论哪种姑息性手术,均不能延长病人的生存期。但如果连这种手术都不做,病人当然死的更快,只是未必死于肿瘤本身,而是死于肠梗阻或胆道梗阻导致的黄疸中毒。 由于手术切除率较低,故化疗在胰腺癌综合治疗中就显得较为重要了。新近的荟萃分析结果显示,吉西他滨联合铂剂在选择性的应用时,可以延长胰腺癌患者整体生存期 。另外,GV1001作为一种末端转移酶肽疫苗,在进展期胰腺癌的治疗中也引起了学者的关注,有报道称其在治疗不可切除的胰腺癌时,患者中位生存期为8.6个月 。于是有学者想到,如果将二者联合应用,能否起到更好的效果呢?遗憾的是,临床数据初步分析的结果已经显示此联合化疗方案没有作用 。在对付这类复杂凶险的疾病时,1加1未必等于2。 看来,彻底攻克这一医学的顽固堡垒,似乎还在很遥远的将来。更令人憋闷的是,由于病因不清,也没什么有效的预防手段,比较肯定的数据仅仅是吸烟者较不吸烟者发病危险增加,50岁以下者,发病危险增加7倍以上,吸烟者的发病年龄较非吸烟者提前十年,因此戒烟应该算是预防措施之一 ,聊胜于无吧。 其实,人类对癌症的恐惧,不过是源于对死亡的恐惧,然而死亡不可避免,因此任何能够延长人类寿命进步都将获得人类极大的重视,2010年美国Lasker大奖的获得者Napoleone Ferrara,其主要贡献就是发现了AVASTIN的抗肿瘤作用,奖金为$250,000。该奖项是医学界仅次于诺贝尔奖的一项大奖,1946年,由被誉为现代广告之父的美国著名广告经理人、慈善家阿尔伯特拉斯克(Albert Lasker)及其夫人玛丽沃德拉斯克(Mary Woodard Lasker)共同创立,旨在表彰医学领域作出突出贡献的科学家、医生和公共服务人员。那么AVASTIN到底有多牛逼呢?2004年美国FDA批准了AVASTIN用于治疗转移性结直肠癌,联用AVASTIN的患者,其生存期可延长5个月 。因此,假如何教授的成果能够被确认的话,获得该奖简直如探囊取物,当然25万美金对于何教授这样的中国名医来说算不得什么,可是,此举毕竟是个可以使中医扬名立万走向世界的大好机会,功在当代利在千秋,万望何教授斟酌,怎么能使这么好的疗法仅仅造福你自己的患者呢?而且,该奖项素有美国诺贝尔奖之称,很多学者均在获得Lasker大奖之后,又获得了诺贝尔奖,何教授是否也能一箭双雕呢?今年诺奖委员会的某些人,让中国一部分人不太爽,如果何教授名至实归地可以夺得诺贝尔医学奖,那会让中国人爽死的。虽然,我未能查到何教授在任何正规学术文献上提到对近400例晚期胰腺癌的纯中医治疗中,3年生存率超过了40%,但我相信,这一定是在搜集和整理数据,既然博客上已经提前透露了这个讯息,我们还是翘首以待吧。 参考文献 1、 复旦大学上海医学院主编,《实用内科学》第12版:1279(2005年7月出版) 2、 廖 泉,朱 预. 胰腺癌外科治疗的现状与展望,中 国 医 学 科 学 院 学 报,2005,27(5):556-559 3、 Jemal A, Siegel R, Ward E, et al. The results contradict a previous study in 2008 that suggested a link between pancreatic cancer and previous hepatitis B infection. Hepatitis B is an inflammation of the liver caused by a viral infection. Study results will be presented at the American Association for the Study of Liver Diseases' Annual Meeting in Boston. Using data from Henry Ford Health System, physicians looked at more than 74,000 patients who were tested for hepatitis B between 1995 and 2008. In the overall analysis, only age was found to be a significant predictor for pancreatic cancer. We looked at the incidence of pancreatic cancer among hepatitis B-infected patients over a 13-year period and found that we could not confirm a higher risk for those with a previous exposure to hepatitis B, as a prior study suggested, says Jeffrey Tang, M.D., gastroenterologist at Henry Ford Hospital and lead author of the study. When other factors are considered such as age, race, sex, HIV status, and the presence of diabetes only older age and presence of diabetes proved significant, whereas prior exposure to hepatitis B was no longer an important variable. According to the National Cancer Institute, more than 35,000 people in the U.S. die of pancreatic cancer each year and 42,000 new cases are diagnosed. The survival rates for patients with pancreatic cancer are poor. An estimated 800,000 to 1.4 million people have chronic hepatitis B infection, according to the Centers for Disease Control and Prevention. In 2007, an estimated 43,000 people in the United States were newly infected with hepatitis B, although many cases are not reported because many people do not have symptoms.
