[转载]南医大抗癌博士秦超：毕业季献唱 “好歌曲” 诉说人生

秦超 是名外科学博士，34岁的他有张娃娃脸，鬓边有一些和年龄不相称的白发。刚刚结束鼻咽癌的治疗，却坚持完成了5万字的肾癌研究博士论文。已发SCI论文12篇、meta分析3 篇、中文期刊 8 篇，主持 1 项国家青年自然科学基金，拿到 4 项国家和省部级课题、3 项国家专利，校优秀研究生奖学金一等奖、2013 年度研究生国家奖学金……秦超专业做得不错，同时也是南医大一附院泌尿外科的手术好手。音乐不是他的本行。 “我的大学，一半是青春，一半是热血。但在南医大读博期间，生病之后萌生把兄弟三人的音乐作品制作成专辑的想法。”现在一有空，秦超就在书房抱着吉他对着手机录歌曲小样，然后在电脑上一遍遍听。个人出资 8万，计划出 500 张CD。“虽然知道铁定亏本，但这是我梦想清单的一部分。”他说：“因为每首歌，都是我的一段 人生 路。” 菁菁木兰洲，悠悠红气球 “长夜空虚鸣蝉扰人梦，星汉西流夜醉木兰洲。今宵一壶浊酒千杯忧，化入堤上沉沉柳梢头。”——《木兰洲》 这首《木兰洲》是他们在南医大读大二时写的。中国风，听起来颇有《中国好歌曲》里《卷珠帘》的韵味，歌名是他老家附近小公园的名字，看上去很文艺。最近他又把它翻出来，加入电子摇滚元素。他说自己是一个向前看的人：“老歌要有变化，新歌要反映现在的想法。” 高三那年，其他同学都埋头苦读时，他用140元买了把最便宜的吉他，花10元买本教程。三天后，他能像模像样地弹唱沈庆的《青春》。 “大学时代组过乐队，开过专场，篮球、足球、羽毛球、网球都能玩玩，还写写小说，当年肚子也不大。”他笑。读本科时，他常去一家名为红气球的酒吧演奏，还专门写了一首歌就叫《红色气球》——“曾经的舞台，曾经的歌声，曾经的姑娘，曾经的少年，都消失在街头。” “我怀念以前的日子，但就像歌词里面唱的，红气球渐渐飞远了。”他说：“我喜欢很多东西。每样都可以拿到 90 分，但再往上就难了。实习后就想纠正这样的状态，放弃了包括音乐在内的不少爱好，一门心思扑到临床上。” “那时我不要 90分，一心想要100。”为了提高技术，秦超经常“蹭开刀”。实习期间住医院旁的宿舍，“买通”了宿管阿姨，能在晚上溜到值班医生那儿，通过帮忙写病历、干杂活换取上台动手的机会，上级医生坐镇，他动手能力提高很快，“业务”也繁忙。 “学习就是这样，得主动。开刀、科研、写歌编曲，都需要反反复复练。”2006年，秦超从南医大毕业，获硕士学位，8 月进入南医大一附院泌尿外科工作，工作4年后攻读博士。 一切都很顺利，直至一场突如其来的疾病。 人生路上，边走边唱 “唱支励志的歌谣，总好过阵阵的叹息。有梦的日子已最美丽，为名利太多的猜忌。我宁愿潇洒地放弃，折断了翅膀又有什么意义?”——《人生路上，边走边唱》 2011 年，他觉得耳朵憋气，听力下降，活检报告显示他患上了鼻咽癌。 看到结果的第一时间，他不知该如何反应，甚至有些木然。当天中午岳母来电话，七个月大的儿子在电话里喊了第一声“爸爸”。 “眼泪这才流下来。”秦超说，对病魔的抗争就此开始。 “三个月不能吃喝，靠输液撑着，眼瞅着一袋袋营养液灌进自己身体里。”三十几次的放疗、化疗和分子靶向治疗，表皮生长因子拮抗剂可以阻止肿瘤生长，但嘴里因放疗引发的溃疡也因此无法愈合，痛到几天几夜无法入睡。 “镇痛泵没用，我只能坐着，感觉嘴巴里像有人打架。”想喝饮料，父亲下楼给他买了瓶“尖叫”，他下定决心要喝但还是因为疼痛没成功，抱着瓶子闻了好几天。 最后他想了个辙：有种麻药叫丁卡因，泌尿外科用它来做粘膜麻醉，但毒性大。毒性再大也要睡觉啊，就用丁卡因稀释液漱口、吐掉，然后睡，半小时后疼醒，再漱口、再睡……停止饮食的30 天后，嗅觉变得异常灵敏，他隔着两张病床闻到了阳台上病友家属手中黄瓜的清香。后来实在想吃，家人把鱼圆压烂了做成糊糊给他喝，也是一样丁卡因漱口麻醉，吃小半碗，疼了再漱口，再吃。“后来还能吃点西瓜，不过入口我就觉得是咸的。”味觉也混乱了。 最大的痛苦来自绝望，病情在治疗一段时间后，并没像预期那样好转。中学时代一起玩音乐的发小“鱼果”送了他一把小吉他，坐在床头，轻抚琴弦，音乐让他渐渐远离悲观和消极。“人是铁饭是钢，再有得玩，情况就好一些。” 人生路上，边走边唱。秦超说这首歌送给以前拼命的自己、送给正在折腾的兄弟们，以及自己的一位良师益友。他形容以前自己是拼命三郎：一天门诊病人 100 多；最长的手术从上午 8：30开到晚上11：30，全靠护士从口罩边塞几块巧克力到嘴里撑着；白天开刀晚上看文献、写论文，两三点钟睡觉是常事。 “大伙儿都稍微松口气，歇歇。” 为青春歌唱 “你厌倦了吗? 你是否也想逃?带上你正燃烧的青春跟我浪迹天涯，带上你对自由的向往跟我走吧，就带上你最爱听的歌谣跟我去那远方。为年轻歌唱，为自由歌唱，为青春歌唱。”——《青春之歌》 2013年1月，结束治疗的秦超恢复门诊并继续读书。 “第一天上班，第一个词：久违，第二个词：心虚。主要担心精力顾不上。”他说，第三个词是兴奋，不是高兴。 “坦白讲，5 年内30%的生存率像块石头压在我心上。工作对我来说，是多年努力的价值体现，也算是帮病人，要对得起他们。” 师弟钱健告诉记者：“跟着超哥门诊时，我们发现他对病人非常耐心和用心，这些都影响着我们。” “超哥喜欢新东西，喜欢去尝试新的技术，想法多、创意也比较新，并且能够付诸行动。”钱健说， “带教过程中，超哥给我们讲解疾病系统、全面，从症状、体检、辅检到治疗都讲到，每次跟他门诊都会获益良多。” 秦超的科研工作，以“解决实际问题、对临床有帮助”为目标，同事居小兵对他的评价是：“为我们科室的基础与临床研究注入了营养液。” “我研究的，是根据每个人的遗传信息，区分出哪些人容易得肾癌，帮助临床早期诊断、评估患者的预后，并根据每个人不同的遗传特点选择最佳的综合治疗方案。”2012 年6 月重新投入工作，他发现在中国人群中，多个基因与患泌尿生殖系统肿瘤的风险有关，而筛查另一些基因并进行联合分析，可以对患者的预后和药物敏感性进行评估，并为患者选择个性化的治疗方案。 显然，2011 年的这场疾病没有打败他，2012 年，他获得主持国家自然科学基金项目，江苏省卫生厅医学新技术引进奖二等奖、国家专利两项、南医大优秀研究生奖学金一等奖，2013 年项目获江苏省科技进步一等奖、江苏省卫生厅医学新技术引进奖一等奖……成绩单依然洋洋洒洒。 但音乐的梦想从没远离，心中的红气球从未飘远。“当年的三人组，我当了医生，鱼果做了编辑，只有蚂蚁还在音乐的道路上坚持。”他决定着手打造音乐专辑，为了蚂蚁，也为了自己。 重拾音乐，学会珍惜 “你用你最好的光阴，陪伴我受伤的日子。年轻的时候，不知道为你写首歌，或是被你宠过了头。时光飞逝如流，不变的是你的温柔，怎会忍心再看你的哀愁。”——《如果没有你》 写本真实专业的医疗小说；出一张音乐专辑；翻译一本书；去埃及、布拉格、耶路撒冷、曼彻斯特；在芝加哥联合中心球馆乔丹雕塑下拍张照。最重要的，带老婆去人迹罕至的海岛度假——这是他真实的梦想清单。 “最近我给老婆写了首歌，叫《如果没有你》。”秦超调皮地说，因为夫人是专辑的投资方，所以必须写首歌来讨好她。 “我生病的时候，她用湿毛巾帮我敷嘴巴保持湿润，结果一夜下来嘴边都破了皮。”秦超说他们之间没什么感人桥段，常有的倒是夫人好心办笨事儿的笑料，但就是靠着她——他在《我的故事》这首歌里称她“我的姑娘”——一天天地医院家里两头跑，不离不弃地撑着他们共同的生活。 养病期间，秦超还“顺便”翻译了一本25万字的小说《单身父亲》。他介绍，书中描写了很多好爸爸。“书送给儿子。希望到他能看懂时，我还在。” 因为还在恢复期，秦超现在只上一些小手术。到了下半年，预计大手术就可以“解禁”。“我喜欢拿手术刀。从刚工作时的四类手术，慢慢做到可以开一类手术，就好像是修炼的过程，最终成为科室老师们那样的高手。”他说：“只是现在心态更好。” 一有时间，他还要去管理自己设在“好大夫在线”的个人网站。解答的问题包括泌尿系统肿瘤、男科疾病、泌尿系统结石、前列腺疾病，甚至心理问题。现在网站访问量有95 万次，他累计回答了 1400 余例患者的问题，贡献值27674，满意度100%。这块他一直坚持着，甚至在治疗期间也没有放弃。 “学会珍惜，也包括珍惜患者。”在《中国梦想秀》节目里，由肺动脉高压病人组成的“蓝嘴唇”组合唱了秦超的《梦想清单》，“最初的歌词是哥们调侃 2012 世界末日的，生病后我改填了词，比较励志。”歌曲被同为病患的蓝嘴唇们选中并演唱，秦超很高兴。 后记：我的故事 “我很想唱一首歌，把我所经历的、心里所想的都唱出来。可是我想了很久，也没有找到合适的旋律。于是我决定就把这些当成一个故事说说吧……”——《我的故事》专辑最后的这首《我的故事》，只有念白，却没有演唱。童年到现在的成长经历，秦超用着他的南方普通话，娓娓道来。 “我对音乐的态度也有变化。”秦超以前比较重视作曲，但现在都是先有了词才谱曲。“不再是年少时的风花雪月、为情所困，总觉得那有强说愁的意味。”秦超说，现在虽然仍属玩票，但音乐已从爱好变成了直抒胸臆的载体，“也许是因为对人生有了更多感悟吧。” 纯独白的《我的故事》，秦超只录制了一遍。没有新闻联播那样字正腔圆的普通话，不是广播级的设备，但他坚持只录一遍。 “如果只有三十天，你有多少愿望要去实现?如果还有三十年，你又能完成几件?写一张梦想清单，趁青春还没走远，把年少时做过的梦，好好再做一遍。”秦超最喜欢这首《梦想清单》中的歌词：“看从前全部被工作和应酬填满的日子，回顾起来却是一片模糊。生存?还是生活?我选择后者，家人、职业、患者、音乐，皆以真诚相待。”

中国式化疗：逆向而行

前几天，就凌志军先生的《重生手记》，《沈阳晚报》采访了我，以中国式抗癌为题，发了一组通讯。采访中我的言语比较激烈，但似乎问题并没有说得很明白，故又来发一通议论，以作补充！ 就像中国股市老是逆世界经济景气而反向行进一样，国内，对于化疗的认识也似乎在逆向而行。国外化疗界越来越强调有所节制，少用为妙之际；中国用化疗对抗癌症的战争却方兴未艾，似乎刚刚成就为如火如荼之势！反差之大，令人惊讶！举例而言，2012年年底，国内一份著名周刊要做一期关于癌症的专刊（已经刊出），涉及甚广，从基础、流行学、一直到临床，洋洋洒洒六七万字。两位主笔是医学局外人，为了写成这篇专刊，做足了功课（前期准备），了解了大量第一手国外新资讯；接着采访了数十位国内外、中西医专家（甚至包括民间医师）。最后采访的是我，足足电话谈了两个多小时。谈到最后，主笔说了他实在不理解，前面采访接触的诸多化放疗专家，似乎思路与他们所了解的国际趋势大相径庭——国际上对化疗评估越来越低；我们还雄赳赳、气昂昂地试图在这些（指化放疗）领域想大干一场；国际上对癌症根治性观念已经声名狼藉，我们好像还很有市场…… 对此，笔者早已有所意识，但没有谜底！也许，这也是中国特色吧！ 有人可能会诘问：国内化疗用的基本上都是欧美标准方案，药物也部分是（至少有钱人用的多数是）国外进口的！这还会有错吗？本人不是化疗专家，只是临床经历及观察多了，正反两方面看多了，站在一个既局内（肿瘤治疗界）、又局外（非西医医师，没有利益瓜葛）的立场，深切地认为，与欧美新进观念相比较，也许要差上1/4世纪！我们总体上似乎还停留在80、90年代之交的水平！也许，我的话有人会说带有学科偏见！那么，我们看看《众病之王·癌症传》作者悉达多·穆克吉的原话总结，他的这本著作可被认为是权威性的！关于化疗的西方新理念，他说到：“亟待解决的问题 不是 化疗的剂量 要多高 ，而是 要多低 ；有试验正在研究是否把原有治疗规程按比例 减小 ，用 毒性较低的温和剂量施药 ，可以达到同等的治愈率。”换句话说：适度、小剂量，见好就收；而不是像今天中国的那样，越用次数越频；剂量越来越大；组合的药物也越来越多！ 没错吧？管见略同！ 最后，还是要回答我们为什么要反复告诫：对试错中发展起来的化疗，要谨慎的理由——其实，美国九十年代的反思中已经涉及这一点了——人们对于化疗所报的期望值太高了，就像买保险，总认为买得越多，越安全！其实大错特错！老百姓有这个想法可以理解。但事实证明这是虚空的期望——被化疗所救的，与冤死于化疗的，完全不能打平！ 注意，我讲的是冤死于化疗的——不应该用而用了后致死的！ 在美国，80年代末以前是这样；但别人改了！ 在中国，现在仍是这样，且似乎有越来越甚之势！ 最近发生的一件与化疗相关的事值得一提：2012年底由中山大学肿瘤防治中心与复旦大学附属肿瘤医院、北京大学肿瘤医院等联合研究后认定：经典的鼻咽癌化疗方案并不适合中国患者。这方案叫“ＰＦ”方案（顺铂＋氟尿嘧啶），是美国人90年代报道的，据说可将晚期鼻咽癌３年生存率提高３１％，被推荐为中晚期鼻咽癌标准治疗方案。在中国用了多年。新近设置对照组深入研究后却发现：患者在无瘤生存率、总生存率、无远处转移生存率及无局部复发生存率等方面均无差异；但毒副反应明显，“化疗明显增加了黏膜炎、骨髓抑制等毒副反应，且延长了治疗时间和医疗费用，因此其并不能使患者获益，不适合中国患者”。 相信这类尴尬的结果以后还会不断出现！

只有持久，才能佳效

经验告诉我们：对于放疗的后续治疗，必须持久，才能有佳效。 言女士，一位退休音乐教师，1998年患鼻咽癌伴颈项淋巴结转移，放疗后就一直在笔者处行中医药治疗。由于她端庄素雅，因为职业关系，放疗后还经常喜欢清嗓子，故印象深刻。初起，她声音嘶哑，口干舌燥，伴便秘、失眠，此乃放疗“火毒”灼伤之故。约2年后，这些症状大部分都消失了，她说她基本恢复了放疗前的状态。2003年底，她出国定居了，失去了联系，也未再运用中医药善后。 2007年夏季的一天，她突然出现我的门诊，人我已无法辨认。因为放疗“火毒”的后续反应，脸部肌肉已变形，嘴角开张不利。但从名字上，我还是依稀记得她当时的情况。她嘶哑地说（很多情况下，与其说口说，不如说用笔比划着），原本以为放疗毒性3～5年就过去了，不用用中医调理了，故去女儿处定居了。想不到，当时恢复得这么好的情况，却因放疗对组织损伤的持续存在且加剧，正退邪进，现别说发音困难，面部畸形，极端口干舌燥，就连喝口水都不行，都呛得厉害（放疗灼伤了食管、气管平滑肌和括约肌之故。这些组织瘢痕化后，常常机体自身无法修复，只会加剧）；已无法再喝中药了：一因呛，二因全家已移居海外。只是有点后悔当初出国的决定！如坚持中医药治疗，当时恢复得多好啊！原来她是趁回国之际，专程来看看我的。看到变了形的容貌和如此之糟的生活质量，笔者心里有说不出的滋味！ 笔者的经验，放疗与化疗不同。化疗的副作用，常短期内达高峰，以后可长期持续存在，但大多由于机体自身可修复之故，副作用常逐步递减。而放疗呢，多数人初起不明显感到，2～3个月后始感明显；且可能由于灼伤组织的瘢痕化并不断收缩、僵化，无法自我修复，会持续进展，引起周边未灼伤组织的相应反应，少数人一直发展到变成“铁面人”一样。鉴此，对于放疗副作用的防范，必须自第一时间开始，而且要持续不断运用中医药及大剂量维生素C等和新鲜果蔬之类康复手段。否则，难免出现上述不幸。 笔者脑海里一直有个很凄惨的人影，武汉人，笔者见过他2次，但种种原因，未坚持中医药治疗。见他时看到他的脸，我想起了《夜半歌声》和《铁面人》两电影中的主角。他的脸放疗灼伤得比《夜半歌声》男主角有过之无不及，恐怖万分，而无法张嘴、眨眼，一动即抽痛。看见我，只见两眼充满泪，面部抽搐，欲说无法。老伴说，他5年前患鼻咽癌，放疗后复发了，追加了1次；当时医师说好了没有治疗方法，也不需要治疗了。我们当时看也就是口干，放疗处皮肤红而已，故也不懂，一直未治疗。没想到后来竟越来越重，现想治也难，想活都难！吃饭喝水都是我每日用小匙，一匙一匙嘴角边塞进去，一天中光喂饭，就要6～7个小时。他多次寻死，都没死成。看着这般生不如死的凄惨状况，笔者无言而心酸。也许放疗副作用谁都无良策彻底解决，但笔者上百例临床案例中还真没有严重到其1/10地步的？因为他们大都在第一时间接受了中医药的康复治疗。 中医药康复治疗对阻断或减缓放疗副作用的发生发展、部分恢复其正常功能的作用是毋庸置疑的。问题只在于越早介入越好，使用时间越长越安全。

[转载]我国科学家推翻鼻咽癌治疗国际标准方案

　　“挑战权威往往比发现新东西更难。” 　　近日，对记者谈起不久前在国际期刊《柳叶刀—肿瘤学》上发表的鼻咽癌研究结果时，中山大学肿瘤防治中心教授马骏不由作此感慨。 　　历时6年，马骏和他的团队终于证实：国际通行的晚期鼻咽癌标准治疗方案PF辅助化疗不能使患者受益。而且，他们还发现了鼻咽癌预后标志分子，为个体化治疗奠定了基础。 　　临床医生的困惑 　　鼻咽癌中晚期患者的标准化治疗方案，源于1998年美国的治疗指南，即同期放化综合治疗时，辅以3个疗程的PF方案辅助化疗。 　　然而，这个号称可以提高31%生存率的方案，却给患者带来了极大的痛苦和沉重的负担。其毒副作用引发胃肠道不适、咽膜炎甚至肾功能衰竭而威胁生命，导致40%以上的患者无法按计划完成治疗。同时，该方案治疗时间增加3个月、费用增加2000美元。 　　这就是该课题组核心成员之一的陈磊反复提及的“临床医生的困惑”。 　　但是，由于标准治疗方案的权威性及临床试验工程的浩大，国内外一直没有团队敢于挑战这一方案。 　　马骏课题组因此成为这一领域“第一个吃螃蟹的人”。 　　把权威方案拉下马 　　这一契机来自于中山大学的“5010计划”，马骏的团队得到了该计划的全额资助，他们得以放开手脚、挑战权威。 　　作为国际最大宗局部区域晚期鼻咽癌病人前瞻性的随机试验，课题组将508例病人随机分为试验组与对照组。试验组为同期放化疗加3个疗程的辅助化疗，即采用标准治疗方案；对照组则单纯使用同期放化疗。 　　经过近38个月的随访，课题组发现，两组病人在总生存率、远处转移和局部复发上并无显著差异。这表明，PF辅助化疗并不能提高患者的生存率，反而产生毒副作用，增加治疗成本。也就是说，作为晚期鼻咽癌这一群体，并不能从辅助化疗中整体受益。 　　然而，挑战权威并非易事。在论文评审之初，这一结果便受到过质疑。 　　但是，马骏他们最终以科学可靠的数据折服了评委。该论文被确定为“必读类医学文献”，属于医学文献中少于2%的顶级文献。该研究也开始影响鼻咽癌临床治疗，欧洲医疗指南已决定不再采用PF辅助治疗。 　　通行14年的标准治疗方案终被推翻。 　　个体化治疗迈出第一步 　　有“破”还须有“立”。中晚期鼻咽癌的预后效果差、复发转移几率高等难题，仍困扰着每个成员。 　　马骏表示，鼻咽癌个体化治疗方面明显落后于肺癌等，主要原因在于无预后标志物。寻找标志分子，构建良好的预后模型迫在眉睫。 　　经过3年多对465例病理的研究分析，课题组发现了一种由5个miRNA构成的分子标签能很好地预测鼻咽癌患者的预后。也就是说，如果患者被发现携带该标志物，则复发和转移的危险性较高。课题组根据该分子标志物与临床医学研究，构建了一个新的预后评分模型。实验证明，它对患者危险性的预测能力明显优于现有的单纯TNM临床分期。 　　预后标志分子的发现，促使鼻咽癌治疗从循证医学模式向个体化治疗模式转变。这意味着，鼻咽癌治疗可根据患者的个体性差异，筛选出复发转移几率高的个体，进行针对性治疗，预期可将中晚期患者的生存率提高10%以上，而检测费用只需800元。这是鼻咽癌个体化治疗迈出的“第一步”，为未来相关靶向药物的研究开辟了新思路。 　　马骏透露，相关靶向药物已经获得国家食品药品监督管理局的批准，正在进行临床一期试验，目前情况乐观。（本报记者 李洁尉 通讯员 王泽冰 黄爱成） 来源：健康报网

二个月前，赵先生得了鼻咽癌，一直在一家大医院接受放射治疗。最近，医生告诉他肿瘤已得到基本控制，可以回家休息，定期来医院复查。可是，离开医院的赵先生一点也高兴不起来。因为，肿瘤虽然被控制了，但他的双颊发黑，口干舌燥，口腔溃疡，最痛苦的是张嘴受限，上下牙之间只能伸进一个手指尖。这些症状从放疗开始后便逐渐加重，他曾向主治医生诉说他的痛苦，但医生说没关系，这是放疗的反应，放疗病人都是这样。现在放疗结束了，这些并发症也加重了，连吃饭、喝水都困难了。无奈之下，赵先生来到上海源生中医内科门诊部，接待他的中医师告诉他，这些都是放疗的并发症，要是能在放疗的同时，用一些中药，治疗起来就比较容易，症状也可以得到完全控制，不会发展到现在这么严重。随后，中医师给他开了埃克信及几副中药。经过较长时间治疗，赵先生现在已经症状改善。 　　 中医治病在我国源远流长，也有着深厚的民族基础，更由于它在癌症治疗中有相当的疗效，在许多方面可以弥补西医之不足。因此，在临床上应用十分普遍，它可以明显改善患者的生存质量、临床症状以及在很大程度上能使肿瘤缩小甚至消失。中医药对放化疗有减毒增效作用，使用中药防治放、化疗毒副反应是中医治疗肿瘤的一大特色，不但能顺利完成疗程，还有明显减毒增效作用，延长肿瘤病人的生存期。作为综合治疗的一部分，中药与手术治疗、放疗、化疗等同时应用，其目的是减轻放疗、化疗的不良反应，使患者能较顺利地完成疗程，在手术治疗、放疗、化疗之后的中药应用，目的是提高远期疗效。 　　手术前使用中药为手术的切除作准备，可以改善患者的某些脏器功能，以及改善患者的体质；术后使用中药，目的是恢复患者的体质，改善或减轻术后的不良反应，试图避免或减少转移及复发，提高远期疗效。在放疗过程中针对所出现的不同症状给予辨证用药可以减轻放疗的不良反应；放疗后服中药可以提高远期疗效、减少复发与转移。化疗期间服用中药可以减少化疗的不良反应，如消化道反应常用扶正、理气、补肾等为主的药物；化疗间歇使用中药可以改善患者体质，为下一次化疗作准备；化疗后服中药则以提高远期疗效为主。 对不适合手术治疗、放疗、化疗的癌症患者，中药作为主要的治疗方法，其目的是尽可能控制癌症，同时改善症状和提高生存质量。对某些终晚期癌症患者，使用中药的主要目的是减轻症状，在一定程度上改善生存质量。中药还可以与免疫治疗相结合，中药本身具有提高免疫力功能的效果，它与免疫抑制剂联合应用，可以明显提高疗效。 在肿瘤的治疗过程中，科学合理的中医药可以而且应该贯穿始终，没有明显的毒副作用。但患者也不能单纯依靠中药治疗，对能手术切除，以及放、化疗有明显效果的肿瘤应在以上治疗的基础上配合使用中药，采取综合治疗手段，以获尽早痊愈。

[转载]鼻咽癌：远离辛辣腌制是关键

在全世界大部分地区，鼻咽癌较为罕见，但在我国南部沿海和周边，以及从这些地区移居国外的人群中，鼻咽癌的发生率较高。据临床观察，广东地区的鼻咽癌高发，所以鼻咽癌又被称为“广东癌”。 咸鱼、腌制品是病因 世界癌症研究基金会在新版《食物、营养、身体活动和癌症预防》的中明确指出：广东类型咸鱼很可能是导致鼻咽癌的原因之一。 广东类型咸鱼制作时使用的盐较平常少，然后在室外相对较高的温 / 湿度条件下发酵、晒干。研究认为，广东类型咸鱼中高含量的硝酸盐和亚硝酸盐可能是摄入咸鱼后促使该病增加的部分原因。而“广东癌”之所以在广东地区高发，与当地居民喜食广东式咸鱼关系极为密切。 笔者在广州、深圳讲座时，亲身见证了广东鼻咽癌高发的事实。讲座结束，很多患者前来咨询关于鼻咽癌的饮食问题。其中有一名患者让笔者记忆深刻。该患者姓张， 40 多岁，是深圳福田区的公务员。鼻咽癌康复 8 年了，现在康复得不错，还经常去做义工，服务于社会，和笔者已成了好朋友。张先生和笔者一起吃饭，他告诉笔者，广东这里有吃咸鱼的习俗，自己从高中开始，就几乎每天吃咸鱼、咸菜。 我国地域辽阔，民风和民俗差异也很大。由于为了适应当地气候特点、食物结构特点以及生活习惯等原因，很多地方都形成了各自所特有的饮食习惯和风俗。但有些不健康的饮食习惯，我们必须加以改变。特别对于肿瘤患者，尤为重要！ 吸烟诱发鼻咽癌 笔者有这样一位患者，年逾 60 岁。患者告诉笔者，他自己从 18 岁就开始吸烟，香烟已经成为他 40 多年来最亲密的“伙伴”了，后来已经到了每天没 2 包烟不过瘾的地步。 1 年前被诊断出患了鼻咽癌，很明显，这和他吸烟太厉害有一定关系。 研究发现，吸烟会诱发鼻咽癌。重度吸烟者发生鼻咽癌的危险性比不吸烟者高 2 ～ 4 倍，并且和吸烟的量及吸烟的持续时间成正比。当烟雾进入肺，再从鼻腔喷出时，刺激了鼻黏膜上皮，导致黏膜上皮不典型增生、蜕变，当这种增生与蜕变难以控制时，便促使形成了肿瘤。长期处于被动吸烟的环境，也有这种倾向。 所以，为了自己和他人的健康，请尽快戒烟！ 垃圾食品作的孽 有一个现象必须值得重视：垃圾食品有可能导致鼻咽癌；而且，往往这些患者都很年轻。跟何教授一起门诊时就碰到过多例，大多只有 20 多岁。 何 老师对非高发地区年轻的本病患者，往往会很有经验一追问，大都是喜欢吃肯德基、麦当劳、匹萨之类食品的。有个案例很典型。 这个女孩是无锡人， 22 岁， 14 岁就到了英国去留学。家庭条件特别好，在国内时中餐就一直食用肯德基、麦当劳；到英国留学以后，更是肯德基、麦当劳等这些“垃圾食品”当成主食了。到 20 岁就发现生了鼻咽癌。 “过量食用油炸类食品，摄入的脂肪过多，会导致营养不均衡。长此以往，会对身体健康造成不利影响。”教授是这样说的。因为这些食物都是油炸的、高热量的、高盐的、重味的，有可能导致年轻人得鼻咽癌。这类情况非常常见！所以，应该引起人们的充分重视。 鼻咽癌饮食金标准 1 ．禁烟、酒、辛辣、油炸、烧烤类食物。辛热香燥食物易助热伤津，少食用。 2 ．不吃腌制类食物，特别是广东式腌制鱼。 3 ．放疗期间食物宜软，忌坚硬、粗糙、过烫之物，以免损伤被放疗射线灼伤的口腔、咽部黏膜。 4 ．放疗后引起的口干咽燥，应以生津润燥为饮食原则，如茶叶水、柠檬水、水果汁、葡萄糖液、西瓜汁、橙汁、乌梅汤、绿豆汤、梨汁、橘汁等。可多食用枸杞子、石斛、麦冬、菊花、芦根、甜杏仁、红枣、荸荠、白萝卜、梨、山楂、柑橘等食物。 5 ．免疫功能降低者，可适当选用灵芝、黑木耳、银耳、香菇、蘑菇等菌菇类食物，也可多食用海带、紫菜、瘦肉、鱼类、新鲜的瓜果以及豆类食品等。 鼻咽癌食疗方 1 ．佛手、竹叶各 3 克，煎汤代茶饮用，用于各类鼻咽癌患者。 2 ．茅根芦根茶：用鲜茅根和鲜芦根各 30 克煎汤代茶饮。 3 ．寒水石 50 克、竹叶 15 克、银花 20 克，三味一起煎汤，用此药汁煮粥食用，适用于鼻咽癌放疗过程中。 4 ．决明子 30 克，煎汤代茶饮用，也可含漱，用于鼻咽癌咽喉疼痛者。 5 ．石斛生地绿豆汤：石斛 12 克、生地 15 克、天花粉 10 克、绿豆 100 克。将石斛和生地用纱布包，绿豆加水煮烂，取出药渣，加入适量冰糖及冲入天花粉，分次服用，对于鼻咽癌流涕、流血、头痛或放疗口干燥时均可食用。 6 ．石斛 30 克，竹叶 10 克，煎汤代茶饮用，用于鼻咽癌放疗后口干者。对于鼻咽癌放疗后口干、便秘者，也可用玄参、生地各 30 克，共煎汤，以此汤煮粥食用。 7 ．山药肉片：山药 50 克、瘦猪肉片 100 克，同炒食，对于鼻咽癌有乏力、便稀时较为适宜。（ 本文摘自《生了癌，怎么吃》）

红歌的力量

　“雄赳赳气昂昂跨过鸭绿江，保和平为祖国就是保家乡……”当针头扎进血管的那一瞬间，脸色苍白的李剑秋感到钻心的疼痛。躺在化疗台上，她唱起了《中国人民志愿军战歌》，声音不大，但很坚定，鼓励自己撑下去…… 　　这是发生在12年前的一幕，当年，只有44岁的李剑秋被诊断为鼻咽癌晚期，在化疗期间，每次都是唱着红歌，挺了过来。 　　化疗完后，原来在单位里就是文艺积极分子的李剑秋开始组织身边的人唱红歌、跳红舞。“红歌对我影响很大，真的能鼓舞人心，也让我一直坚持了下来。”
