Sharp Recent research into mutations of the KRAS gene in metastatic colorectal cancer demonstrates that the presence of wild-type (non-mutated) KRAS in tumors is correlated with more-effective treatment with the anticancer agent panitumumab. Similar results have been obtained with the drug cetuximab. Such findings hasten the promise of personalizing cancer treatments for specific tumor types. Jablons, Extract | Full Text | PDF Participants who were at an increased risk for lung cancer were enrolled in a trial to determine whether CT screening reduces mortality from lung cancer. In the van Klaveren et al. paper, volume measurements and volume doubling times were used to evaluate the noncalcified lung nodules that were detected by CT scanning at baseline and at years 1, 2, and 4 of the trial. top Clinical Pearls Diagnostic Techniques in the Evaluation of Lung Nodules The use of multidetector computed tomography (CT) in lung-cancer screening trials involving subjects with an increased risk of lung cancer has highlighted the problem that the clinician has in deciding on the best course of action when noncalcified pulmonary nodules are detected by CT. Current practice has been to refer participants in these programs for additional diagnostic evaluation if they have a noncalcified nodule that is larger than 5 mm in diameter. The van Klaveren et al. study used software to evaluate a noncalcified nodule according to its volume or volume-doubling time. Growth was defined as an increase in volume of at least 25% between two scans. The first-round screening test was considered to be negative if the volume of a nodule was less than 50 mm 3 , if it was 50 to 500 mm 3 but had not grown by the time of the 3-month follow-up CT, or, in the case of those that had grown, if the volume-doubling time was 400 days or more. Performance of Volume and Volume-Doubling Time for Lung Nodules Among the 7557 participants in the screening group, a noncalcified nodule between 50 mm 3 and 500 mm 3 in volume was discovered in 1451 subjects at the first screening; 518 of the nodules had a volume-doubling time of more than 400 days on a follow-up scan at 3 months. When participants who had nodules with this slow volume-doubling time were considered together with subjects whose scans were classified as negative at the first screening, and the combined group was followed for 2 years, 20 lung cancers were detected. Thus, a negative scan, which included the presence of nodules with a slow volume-doubling time, was associated with a negative predictive value of 99.7%. Figure 1. A : Among subjects in the van Klaveren et al. trial who were at high risk for lung cancer and screened in three rounds of CT scanning and in whom noncalcified pulmonary nodules were evaluated according to volume and volume-doubling time, the chances of finding lung cancer 1 and 2 years after a negative first-round test were 1 in 1000 and 3 in 1000, respectively. top TEACHING TOPIC 2. Mesana, Extract | Full Text | PDF | PPT Slide Set A 55-year-old man presents with a holosystolic murmur of increasing intensity and is diagnosed with mitral-valve prolapse with severe mitral regurgitation. He is asymptomatic but has mildly depressed left ventricular function and mild left ventricular enlargement. Mitral-valve repair is recommended. Mitral-valve repair may result in a better survival rate than mitral-valve replacement, but mitral regurgitation can recur. top Clinical Pearls Epidemiology and Prognosis of Mitral-Valve Prolapse Mitral-valve prolapse is defined as the displacement of some portion of one or both leaflets of the mitral valve into the left atrium during systole. In developed countries, it is the most common cause of chronic mitral regurgitation with a prevalence of mitral-valve prolapse of about 2.5%. The disorder has both genetic and acquired forms, and several chromosomal loci for autosomal dominant mitral-valve prolapse have been identified. The natural history of mitral-valve prolapse is heterogeneous and is largely determined by the severity of mitral regurgitation. Although a majority of patients remain asymptomatic and may have a near-normal life expectancy, approximately 5 to 10% have progression to severe mitral regurgitation. Left untreated, mitral-valve prolapse with severe mitral regurgitation results in limiting symptoms, left ventricular dysfunction, heart failure, pulmonary hypertension, and atrial fibrillation. Spontaneous rupture of mitral chordae may occur, and endocarditis and stroke are serious complications. Figure 1. The Mitral Valve. Figure 2. Mitral-Valve Prolapse. Treatment for Mitral-Valve Prolapse with Mitral Regurgitation No randomized trials have compared medical management to surgery for severe mitral regurgitation due to mitral-valve prolapse. However, evidence from observational series strongly suggests that surgical intervention is beneficial. The goal of surgical correction for mitral-valve prolapse is to restore a competent mitral valve. There are two options for surgical correction of severe mitral regurgitation due to mitral-valve prolapse: valve replacement or valve repair. Mitral-valve replacement can be performed with the use of either a mechanical or a biologic prosthesis. The goals of mitral-valve repair are to obtain a proper line of coaptation on both leaflets, to correct annular dilatation, and to preserve (or repair, if necessary) the subvalvular apparatus. Figure 3. Special Repair Techniques. top Morning Report Questions Q : What is the mortality rate of patients with mitral-valve prolapse and severe mitral regurgitation? A : The mortality rate of persons who have mitral-valve prolapse with severe mitral regurgitation is approximately 6 to 7% per year. Q : What are the drawbacks of mitral-valve replacement? A : There are several drawbacks to mitral-valve replacement. These include the need for lifelong anticoagulation therapy and the risk of thromboembolism with the use of mechanical valves; the risk of prosthetic-valve deterioration and failure when bioprosthetic valves are used; and the risk of prosthetic-valve endocarditis. In addition, if the chordae tendineae are severed during surgery, the ventricular wall is no longer anchored to the valve apparatus, and the tethering effect of the chordae is lost. As a result, left ventricular wall stress increases and left ventricular function deteriorates. There are no randomized trials comparing mitral-valve repair with replacement. However, data from observational studies suggest that mitral repair is beneficial. Q : Which patients with mitral-valve prolapse should be treated? A : Patients who have severe mitral regurgitation with symptoms or with left ventricular dysfunction (ejection fraction, 60%), dilatation (left ventricular end-systolic dimension, 40 mm), or both should be offered surgery. Likewise, asymptomatic patients without left ventricular dysfunction or dilatation but with atrial fibrillation or pulmonary hypertension should be considered for surgery.
