Fornew readers and those who request to be “ 好友 good friends” please read my 公告 栏 first. Dear colleagues, It gives me great pleasure to announce the appointment ofXiao-Li Meng, Whipple V.N. Jones Professor of Statistics in the FAS'sDepartment of Statistics, as the next Dean of the Graduate School of Arts andSciences, effective August 15, 2012. Xiao-Li is a remarkable scholar, teacher, and citizen ofthe University. With Harvard roots dating back to his days as a student atGSAS, Xiao-Li has deep institutional understanding, coupled with remarkableopen-mindedness and a drive to move Harvard forward. His knowledge of the FASstems not only from his role as an engaged and accomplished teacher and mentor,but also from his years of departmental service as a head tutor, director ofgraduate studies, and three-term department chair. He has also been an active memberof the FAS Faculty Council, the Committee on Undergraduate Education, and theGraduate Policy Committee. In addition to his strong ties to undergraduate andgraduate education, Xiao-Li has worked effectively with colleagues acrossdisciplinary and school boundaries. He has served with distinction as a memberof the School of Engineering and Applied Sciences’ Computational Science andEngineering Program, the University’s Institute for Quantitative SocialScience, the Committee on Higher Degrees in the Public Health Sciences, and thesubcommittee on the Biostatistics Ph.D. degree, a collaborative effort withHarvard Medical School and the Harvard School of Public Health. Xiao-Li joined the FAS faculty in 2001 as Professor ofStatistics, assuming his present title in 2007. As chair of the StatisticsDepartment (2004-2010 and 2011 to the present), Xiao-Li has revitalized teachertraining and professional development for the department’s graduate studentsand fostered an energizing and welcoming departmental culture for its students. Xiao-Li has long been a creative advocate for advancingexcellence in teaching at Harvard. He served on the FAS Committee onPedagogical Improvement (2004-2010) and the FAS Task Force on Teaching andCareer Development (2006-2007). His own teaching is exceptional, and he is awarm and inspiring mentor to students. Xiao-Li’s work spans a broad spectrum, from theoreticalfoundations of statistical inferences, to statistical methods and computation,and to a wide range of applications in natural, social, and medical sciencesand engineering. Examples of the applications of his work include complexstatistical modeling in astronomy and astrophysics, quantifying statisticalinformation in genetic studies, detecting and estimating trends inenvironmental and geophysical measurements, measuring disparities in Latino andAsian mental health services, assessing reporting delay in AIDS surveillancesystems, and de-noising and de-mosaicing in digital cameras. For his research contributions, Xiao-Li has received the2001 COPSS (Committee of Presidents of Statistical Societies) Award, the mostprestigious award by five statistical societies to a single statisticianannually for being the outstanding statistician under the age of forty.He also received the 2003 Distinguished Achievement Award from theInternational Chinese Statistics Association (ICSA), and was elected fellow bythe Institute of Mathematical Statistics (IMS) in 1997 and by the AmericanStatistical Association (ASA) in 2004. Most recently, he was the 2010 MedallionLecturer of IMS, 2011 Mosteller Statistician of the Year by the ASA BostonChapter, the recipient of a 2011 Distinguished Alumni Award by FudanUniversity, and a recipient of the inaugural (2012) PL Hsu Award for distinguishedachievements in research and education by a statistician under the age offifty. Receiving his B.S. in mathematics from Fudan University(Shanghai, China) in 1982, Xiao-Li earned a graduate diploma in 1986 fromFudan’s Research Institute of Mathematics. He then pursued master’s anddoctoral work in statistics at Harvard, receiving the M.A. and Ph.D. degrees in1987 and 1990. Before joining Harvard as a faculty member, Xiao-Li movedthrough the ranks at the University of Chicago, as an assistant professor(1991-1997), associate professor (1997-2000, winning the 1997-1998 FacultyAward for Excellence in Graduate Teaching), and professor (2000-2001). I want to take this opportunity to express my sincerethanks to Richard J. A recent study found that combined oral contraceptives (COCs) and, to a lesser extent, increasing age reduced the severity of dysmenorrhea in women aged 19 to 24 years. Ingela Lindh, PhD, and colleagues Agneta Andersson Ellstrm, MD, PhD, and Ian Milsom, MD, PhD, all from the Institute of Clinical Sciences, Sahlgrenska Academy, Gothenburg University, Sweden, reported their findings in an article published online January 17 in Human Reproduction . They used 2 tools, a verbal multidimensional scoring system (VMS) and a visual analogue scale (VAS), to evaluate dysmenorrhea in 3 groups of women at 19 and 24 years of age. The researchers used the population registers from the city of Gothenburg to identify women who were 19 years old in 1981, 1991, and 2001. They picked every fourth woman from the register in 1981, and every third woman from the registers in 1991 and 2001. The women all resided in Gothenburg at the time of birth and at the times of assessment. They were born in 1962 (62 cohort), 1972 (72 cohort), and 1982 (82 cohort) and were first evaluated at age 19 years in 1981 (n = 489), 1991 (n = 523), and 2001 (n = 392), respectively. They were evaluated again 5 years later, at age 24 years. The team sent postal questionnaires with about 40 questions regarding contraception, reproductive history, menstrual pattern, duration and intensity of menstrual pain, need for medical care, ability to work during menses, and information including height, weight, and smoking status. The VMS instructs the individual to rate their pain as none, mild, moderate, or severe. The VAS has the person rate their pain along a straight line representing a continuum of 1 to 10, with "no pain at all" at one end and "unbearable" pain at the other. The researchers performed a correlation analysis to compare the VMS and VAS in all women who completed them at both ages. They found a significant correlation ( r = 0.83; P .0001) between reports of dysmenorrhea by both tools; therefore, they report the VMS results as mean values. The researchers compared menstrual bleeding pattern and dysmenorrhea severity both within and between the 3 cohorts at 19 and 24 years of age. Fewer women in the 82 cohort reported no dysmenorrhea on the VMS than in the other 2 cohorts. Participants in the 82 cohort had a higher VMS score at 19 and 24 years of age ( P .05 and P .001) compared with the 62 cohort, and a higher VMS score at age 24 than the 72 cohort ( P .01). The 82 cohort reported more severe dysmenorrhea on the VAS compared with the 62 cohort ( P .001) and the 72 cohort ( P .01) at ages 19 and 24 years, respectively. Dysmenorrhea-related absenteeism was 31% to 33% at 19 years of age and decreased in all 3 groups at age 24 years to 20% to 23%. The researchers placed the women in 1 of 2 groups: those who used COCs at age 19 years, but not at age 24 years, and those who did not use COCs at age 19 years, but did at age 24 years, and compared these groups with each other. Women were excluded from these analyses if they had given birth, used an intrauterine device, or used progestogen-only contraception methods. When all 3 cohorts were analyzed together, women who used COCs at age 19 years, but not at age 24 years, had increased severity of dysmenorrhea, and women who did not use COCs at age 19 years, but did at age 24 years, had decreased dysmenorrhea severity. The decrease in dysmenorrhea between the cohorts was demonstrated equally by the VMS tool (0.58, confidence interval, 0.35 - 0.81; P .0001) and the VAS tool (18.8, confidence interval, 11.6 - 26.0; P .0001). One study limitation is that few women included in the trial gave birth, which reduced the ability to assess the effect of childbirth on dysmenorrhea. "The study was a large longitudinal cohort study that used patients as their own controls, as it was done over time. The age-related findings may have had a decrease in smoking and increase in weight as confounders, but showed a statistically significant decrease in dysmenorrhea with the use of COCs," Leah Kaufman, MD, residency program director, North Shore Long Island Jewish/Hofstra School of Medicine, New York City, told Medscape Medical News in an interview. "When analysing the cohorts separately, a corresponding decreased severity of dysmenorrhoea was noted for the 62 and 72 cohorts according to both measurement systems, but in the 82 cohort only when using the VAS," write the Dr. Lindh and colleagues. COCs and increasing age reduced severity of dysmenorrhea independent of each other, according to the authors, and COCs reduced dysmenorrhea more than increasing age or childbirth. "The decrease in the severity of dysmenorrhoea achieved by COCs was equivalent to the transfer of every third woman one step down on the VMS scale, which — in clinical terms — will result in less pain, improved working ability and a decrease in the need for analgesics," the authors write. "Effective management of dysmenorrhoea is beneficial for both the afflicted individual and society and thus the possibility of a beneficial influence of COCs on dysmenorrhea should be included in contraceptive counselling," write the authors. Dr. Kaufman agrees. "As a practicing generalist I regularly prescribe COC's for the treatment of dysmenorrhea, with marked improvement in their functionality and a decrease in their absenteeism from work and school," she said. "The potential issue of difference in pain perception and a decrease in pain, particularly with respect to dysmenorrhea associated with age, was suggested in the article, but not proven. Differential pain perception by age and the need for physicians to tailor treatment and counseling based on this concept would be interesting, if proven with the elimination of other confounding factors," she explained. Among patients with chronic kidney disease (CKD) and hypertension, taking at least 1 antihypertensive medication at bedtime significantly improves blood pressure (BP) control, with an associated decrease in risk for cardiovascular events, according to new research. Ramón C. Hermida, PhD, and colleagues from the Bioengineering and Chronobiology Laboratories at the University of Vigo, Campus Universitario, Spain, published their findings online October 24 in the Journal of the American Society of Nephrology . According to the researchers, the beneficial effect of taking BP medication at night has been previously documented, but "the potential reduction in risk associated with specifically reducing sleep-time BP is still a matter of debate." The current prospective study sought to investigate in hypertensive patients with CKD whether bedtime treatment with hypertension medications better controls BP and reduces CVD risk compared with treatment on waking. The study included 661 patients with CKD who were randomly assigned either to take all prescribed hypertension medications on awakening or to take at least 1 of them at bedtime. Ambulatory BP at 48 hours was measured at least once a year and/or at 3 months after any adjustment in treatment. The composite measure of cardiovascular events used included death, myocardial infarction, angina pectoris, revascularization, heart failure, arterial occlusion of lower extremities, occlusion of the retinal artery, and stroke. The investigators controlled their results for sex, age, and diabetes. Patients were followed for a median of 5.4 years; during that time, patients who took at least 1 BP-lowering medication at bedtime had approximately one third of the CVD risk compared with those who took all medications on awakening (adjusted hazard ratio , 0.31; 95% confidence interval , 0.21 - 0.46; P .001). A similar significant reduction in risk with bedtime dosing was noted when the composite CVD outcome included only cardiovascular death, myocardial infarction, and stroke (adjusted HR, 0.28; 95% CI, 0.13 - 0.61; P .001). Patients taking their medications at bedtime also had a significantly lower mean BP while sleeping, and a greater proportion of these patients had ambulatory BP control (56% vs 45%; P = .003). The researchers estimate that for each 5-mm-Hg decrease in mean sleep-time systolic BP, there was a 14% reduction in the risk for cardiovascular events during follow-up ( P .001). According to Dr. Hermida and colleagues, "treatment at bedtime is the most cost-effective and simplest strategy of successfully achieving the therapeutic goals of adequate asleep BP reduction and preserving or re-establishing the normal 24-hour BP dipping pattern." The authors suggest that a potential explanation for the benefit of nighttime treatment may be associated with the effect of nighttime treatment on urinary albumin excretion levels. "We previously demonstrated that urinary albumin excretion was significantly reduced after bedtime, but not morning, treatment with valsartan," they note. Tatsuya Amano, now at the University of Cambridge, UK, and his colleagues examined the influence of topography and climate on the diversity of 230 rice fields in Japan with varying levels of pesticide use. The researchers measured the abundance of pest-eating Tetragnatha spiders as a proxy for biodiversity. They modelled the data and found that fields with reduced pesticide use had more spiders, with the greatest abundance in cold or wet areas. The authors also used their model to predict which regions of Japan might benefit most from organic farming through increasing Tetragnatha numbers. Ecol. Traffic-related air pollution may increase the risk for preterm births, according to the findings of a new study conducted in southern California. Polycyclic aromatic hydrocarbons (PAHs) were especially strongly associated with increased risk, with up to a 30% increase in premature births. Michelle Wilhelm, PhD, from the Department of Epidemiology, School of Public Health, University of California, Los Angeles, and colleagues reported their findings in an article published online October 7 in Environmental Health . According to the researchers, several studies have reported an association between levels of pollutants, including carbon monoxide and particulate matter exposure, and preterm birth; however, only 2 studies have reported a positive association when factoring in traffic levels near homes. The current study sought to examine risk for preterm birth in women from Los Angeles who were exposed to high levels of traffic-related air pollutants prenatally, using "more spatially-resolved exposure models...as well as available data on specific toxics of biologic interest for this outcome." The researchers analyzed data on 241,415 births occurring from June 2004 to March 2006 among women residing within 5 miles of a monitoring station. Of these births, 10,265 preterm births (infants born at 37 completed weeks of gestation) were reported. The number of preterm births was correlated with pregnancy period exposure averages for several pollutants, including PAHs, nitric oxide, nitrogen dioxide, and nitrogen oxides. Each preterm birth was matched with 10 normal-gestation healthy control patients. Three different information sources were used: government "criteria pollutant" monitoring stations (including carbon monoxide, nitrogen dioxide, ozone, and fine particulate matter), a traffic pollution model (Land Use Regression), and data about toxic chemicals collected by the South Coast Air Quality Management District. The risk for preterm birth increased from 6% to 21% for each quartile increase in exposures to organic carbon, elemental carbon, benzene and diesel, biomass burning, and ammonium nitrate (with aerodynamic diameter 2.5 μm; ie, PM 2.5 ), and a 30% per interquartile increase in PAHs. The short time period (ie, 22 months) was one study limitation. According to the researchers, oxidative stress may be "one potential biological pathway of interest" for the influence of air pollution on preterm birth. " potential biologic mechanism through which and PAHs could exhibit their influence on adverse birth outcomes is through acting on oxidative stress and inflammatory pathways during pregnancy," Dr. Wilhelm and colleagues add. "Our results point to PAHs as pollutants of special concern that should be a focus of future studies," they write. "PAH sources other than traffic also contributed to higher odds of preterm birth, as did ammonium nitrate PM 2.5 , the latter suggesting a role for secondary pollutants."
